Patients who no longer have teeth in their arch or are about to lose them can benefit from fixed teeth on implants as an alternative to the classic prosthesis, the one that used to be kept in a glass at night.
Dental implants are inserted to create the premises for the retention of the work on implants and regardless of how it is attached to the implants, the benefit of its retention is clearly superior to a classic total or partial denture.
In the maxilla (upper part), where the bone is more porous, it is advisable to place ideally 6 implants, and in the mandible (lower part) minimum 4 implants to support a prosthetic work on implants.
This type of work restores the aesthetics and functionality of the smile to a level similar to the patient's lost natural dentition.
The most important aspect in these cases, besides aesthetics and mastication, is the patient's access to hygiene, respectively how easily the patient can sanitize under this dental work alone at home.
A work that cannot be sanitized by the patient at home endangers the survival of the implants, due to bone resorption.
The presence of bacteria near the implants creates the prerequisites for infection at the level of the implants with their consequent loss. The process is often slow, and the patient only notices problems 3-5 years after the initial intervention.
If we consider that these implants should last a lifetime, then no compromise is possible when it comes to aspects related to oral implantology.
Depending on the retention mechanism at the implant level, teeth fixed to the arch by implants can be of several types:
- screwed directly into the implant (fixed teeth) all-on-4, all-on-6 and all-on-x
- retention by hydraulic telescoping
- retention by special means of retention, maintenance and stabilization.

- they are usually made on implants fixed in the bone, rarely they have an indication for making them on teeth;
- the prosthetic restoration (work) is attached to the implants by means of pieces with a rigid-elastic connection of the father-mother type;
- they require more frequent maintenance (periodic visits to the doctor) due to the need to change special Teflon pieces that are inside the removable dental prosthesis;
- it is advisable to do hygiene at least twice a day (morning and evening) according to the doctor's instructions.
If necessary, the number of implants required is discussed with the dentist.

- they are fixed, but can be removed at home by you, this aspect being a huge advantage; telescopic dentures are the latest generation of removable dental prostheses;
- you can sanitize the restorations (work) and teeth or implants much more easily;
- they keep the teeth and implants in the arch for longer than fixed ones, because the patient can remove the restoration (work) himself, without the help of the doctor (much easier personal hygiene); - their repair over time, if complications arise, is much easier and less expensive than with fixed ones;
- telescopic prosthesis is the only dental prosthesis (prosthetic restorations) that can be mounted on both teeth and dental implants, fixed ones being contraindicated due to the different mobility of teeth and implants (teeth 50-150 microns / bone implants 15 microns);
- the loss of a tooth or implant over time does not require the restoration to be rebuilt;
- the teeth or implants represent the primary telescope;
- the secondary telescope (galvanized gold of 99.99% purity) is fixed in the restoration (work);

When the situation is favorable, the implants can be loaded with teeth immediately postoperatively, thus the patient receives the teeth in the same session as the implant insertion, the restoration of the smile and masticatory function is achieved immediately, and this aspect is popularly known as teeth in 24 hours, teeth in the same day.
The conditions for having teeth in 24 hours, respectively fixed teeth in the same day, are:
the patient still has bone supply favorable for the placement of dental implants in a correct position
the quality of the bone tissue is appropriate, respectively the bone is sufficiently dense
the position of the implants is favorable for performing a work that can be easily sanitized by the patient
They restore the function and aesthetics of the teeth.
- they are fixed in the oral cavity and cannot be removed by the patient;
- they are performed on implants fixed in the bone; 4, 6 or more implants, as appropriate.
- they cannot be sanitized under the prosthesis. A doctor's help is necessary for complete sanitization and removal of residues accumulated under the prosthesis, on the gums. These residues can lead to inflammation of the gums, and in some cases, to bone loss.
- they are a cheaper option compared to galvano telescopic dentures and more expensive compared to dental prostheses on ball / staple implants.
- they require regular visits to the doctor, every six months, due to the need for hygiene and check-ups.
- personal hygiene is necessary twice a day and must be done according to the dentist's instructions.
